Prefer a faster route? Book your appointment directly — your intake is handled automatically through Tebra, DeluxMed's HIPAA-compliant scheduling platform. The steps below are for patients who have been directed to use this portal as their onboarding method.

Complete Digitally
Open the PDF, click each field, and type your information. Use the Tab key to navigate. Save when complete.
Submit by Email
Email your completed forms to before your appointment. We confirm receipt.

Not necessarily. When you book an appointment through our website, your intake process begins automatically within Tebra — DeluxMed's HIPAA-compliant electronic health records platform. You will complete an entry form during booking, and your provider will send any additional condition-specific forms to your email once your appointment is approved.
This portal is available as an alternative if you have been directed here by our clinical team, or if our scheduling platform is temporarily unavailable. If you use both methods, some information may need to be re-entered within the scheduling platform, as it is the system of record for all patient data.
Recommended: Book your appointment first — it is the faster, more complete onboarding path.
